Note : Never leave column number 4 in form 6 because it needs your last address and you should also tell that you have already a voting card or not.

Document Required to Apply for Voting Card :

- 1 Age Proof

- 1 Address Proof

- 2 Passport Size Current Colored Photo

When you get your voting card and you found any mistake in your name, father’s name or address etc. then fill form number 8 for correction. If you are living on rent then you have to give a photocopy of rent agreement as well.
